  1. Introduction

We are Elevate Physio & Performance Limited, a company registered in England & Wales with registered office at 2nd Floor Gadd House, Arcadia Avenue, London, England, N3 2JU and company number 12204895.

These terms apply to our provision of service to you, and by using our service (which includes by booking and / or attending an appointment) you agree to be bound by these terms.

Please read these terms carefully prior to making a booking with us.

  1. Payment and Insurance Claims

If you are paying for your treatment without claiming through insurance, payment for each appointment is required at the end of such appointment.  Our fees are available on request and may change from time to time.

If you are making a claim from your insurance provider for your treatment, please inform us when making the appointment or prior to attending the appointment.  It is your responsibility to check with your insurance provider prior to attending your first appointment that they will cover your treatment.

Prior to, or at your first appointment, you will need to provide us with your insurance details, including the name of your insurance provider, your membership number and your claim authorisation number.  Failure to provide these details may mean you will be charged in full for such appointment at the end of the appointment.

Provided we have received the appropriate information from you, we will invoice the insurance provider directly on your behalf.  Please note however that certain insurance providers require you to pay us for your treatment and then reclaim the fees from the insurance provider.  Please check with your insurance provider prior to attending your first appointment to clarify their required process.

It is your responsibility to confirm with your insurance provider any restrictions of your insurance cover prior to your first appointment.

In certain circumstances, your insurance provider may reject part or all of our invoice, for example, if you have exceeded your insurance excess, you have exceeded the number of appointments they have authorised, or the cost per appointment exceeds the amount they have agreed to cover for each appointment.   In such event, you are responsible for paying the outstanding amounts and such payment is due within seven days of being notified of such amount.

We reserve the right to charge interest on any overdue amount at the rate of 2% per annum above the Bank of England’s base rate from time to time. Such interest shall accrue on a daily basis from the due date until actual payment of the overdue amount.

  1. Cancellation Policy

If you have made an appointment but are unable attend, please provide us with at least 24 hours’ notice and we will reschedule your appointment if required.  If you fail to attend your appointment without giving us at least 24 hours’ notice, you will be charged in full for the missed appointment.  In addition, if you are late for an appointment, we will, at our reasonable discretion, consider this a missed appointment and you will be charged in full for the appointment.

Please note that your insurance provider is unlikely to provide coverage for missed appointments and you will be responsible for paying our fees yourself.  Any such payments for missed appointments are due within seven days of the date of your appointment.

  1. Referrals

We may refer you onto a third party health professional, for example if further treatment or imaging is required.  In such event, we are not responsible for such third party’s treatment.

  1. Governing law and jurisdiction

These terms and any dispute or claim arising out of or in connection with it or its subject matter shall be governed by and construed in accordance with the law of England and Wales, and the courts of England and Wales shall have exclusive jurisdiction to settle any dispute or claim arising out of or in connection with these terms.
